The ODH Cardiovascular Health (CVH) Program is seeking a contractor to expand an electronic coordinated care referral network (e.g., Unite Us, FindHelp, Wellsky) of health, human, and social services providers in Ohio. This project will simplify referrals to community resources to address the social service needs for individuals at risk for cardiovascular disease (CVD). The contractor will provide support and technical assistance (TA) to healthcare providers participating in a CVH Quality Improvement Project (QIP) on how to utilize the electronic referral platform for referrals and bi-directional communication. In addition, the vendor will link social needs referral data from the referral platform with clinical data to provide statewide data on the number and proportion of individuals at risk for CVD that are referred for resources to address their social needs.
