Offerors are advised that any personal service contract resulting from this RFP must comply with all applicable provisions of KRS 45A and KRS 12.210 prior to becoming effective. A contract, based on this RFP, may or may not be awarded. Any contract award from this RFP is invalid until properly approved and executed by the Finance and Administration Cabinet and filed with the Legislative Research Commission, Government Contract Review Committee. 1.00 Purpose and Background The Thomson-Hood Veterans Center ( THVC). is issuing this Request for Proposal ( “RFP”) to provide medical laboratory services for the residents of its 285-bed long-term care skilled nursing center, located at 100 Veterans Drive, Wilmore, KY 40390. 2.00 Scope of Work Document Phase Document Description Page 5 2600000454 Final 074/THVC/MEDICAL LABORATORY SERVICES Total Pages: 27 #Provide the THVC’s residents with laboratory services as ordered by the attending physician/nurse practitioner and notify the THVC of the test results. #Provide THVC and attending physician/nurse practitioner with the tests results through integration with our PointClickCare electronic medical record platform. #Comply with all federal and state laws and regulations governing laboratory services. The vendor will also notify THVC of any policy changes derived from current state and federal regulations as they affect the laboratory services. #Make all laboratory services available to all residents without regard to race, color, creed, national origin, age, sex, religion, or handicap. #Provide THVC with cumulative laboratory reports if and when they become available. #Provide orientation in-services to THVC directly related to the use of laboratory services such as completing the requisitions and proper collection and handling of specimens. Reference books will also be provided. #All representatives from the services the Vendor provides must have the mandatory backgrounds checks required by the OBRA Regulations 483.13(c)(1)(ii); 483.75(b); 483.75(g)(1); 483.75(g)(2). Vendor is responsible for ensuring none of their employees, representatives, independent vendors have been suspended, debarred, excluded or ineligible to participate in federal health care programs, which includes Medicaid and Medicare. #Billing for all tests shall be handled by the vendor either under Medicare Part B or private insurance according to federal regulations or by billing THVC for laboratory tests provided to those residents who are not covered by Medicare or private insurance or when the laboratory test is not a covered benefit. Under no circumstances will THVC’s residents be billed. Charges to THVC will be at the rate set forth on the agreed upon fee schedule. Services rendered for THVC’s residents and billed to THVC will be billed on a monthly-itemized statement showing date of service, resident ’s name, test performed and extra charges (including extra trip and specimen pick up). Statements for laboratory services billed to THVC will be submitted no later than the second week of the month following the date of service. Medicare part B and/or private insurance deductibles and co-pays will not be charged to THVC. #Provide a schedule for phlebotomy services and specimen pickup. Unscheduled specimen pickup should be available seven days a week. (Include in the proposal any extra fees for specimen pickup and phlebotomy service outside of scheduled service. #Will phone immediately after the test has been run to report any “panic” levels and will also print the results. Provide the THVC’s attending physicians/ARNP of the labs standardized “panic” levels. #Will collect and process all tests and print all results within 24 hours of collection. All tests that cannot be processed during the normal time frame will be reported as soon as the results are available. All “stat” requests will be reported within two hours of receiving the specimen. #Re-testing will be done at the request of THVC when the accuracy of the test result is questioned. #Phlebotomy services will not be any earlier than 5:00 am without the consent of the THVC. #Laboratory will adhere to all THVC policies and review and cover HIPPA/Confidentiality, Resident Abuse , Resident Abuse Reporting, Resident Rights, and Infection Control with staff that will be onsite at least once annually and if any changes are made policies or procedures. Comply with any additional mandatory trainings if required for contractors. Document Phase Document Description Page 6 2600000454 Final 074/THVC/MEDICAL LABORATORY SERVICES Total Pages: 27 #Under the Health Insurance Portability and Accountability Act of 1996 and the final regulations promulgated thereunder, as amended from time to time ( “HIPAA”), comply with the applicable provisions including all data, documents, discussion, or other information developed or received by the awarded Vendor or provided for performance of agreement are deemed confidential and shall not be disclosed by awarded Vendor to any other party than the Commonwealth of Kentucky. #Maintain all required insurances for their employees and facility as long as they remain a vendor of the THVC and provide proof of insurance to facility including annual updates. #Comply with all federal and state vaccination regulations, which includes providing proof of compliance prior to beginning employment and/or providing services. #Cost of travel and lodging will be paid by the vendor and is not the responsibility of the THVC. #Contractor must have ability to integrate/interface with Point Click Care (PCC) Software system. One integration requirement is bi-directional orders and results. 3.00 Evaluation Criteria The Thomson-Hood Veterans Center ( THVC) shall conduct a comprehensive, fair, and impartial evaluation of all proposals. The THVC may reject any proposal that is incomplete or in which there are significant inconsistencies or inaccuracies. The THVC reserves the right to reject all proposals. The THVC has established a Proposal Evaluation Committee to review, evaluate and verify information submitted by the offeror. Each vendor is responsible for submitting all relevant, factual and correct information with their offer to enable the evaluator(s) to afford each vendor the maximum score based on the available data submitted by the vendor. The THVC shall evaluate the proposals by assigning scores as indicated. Will submit a copy of current liability, workers comp, and automobile insurances. Will submit a list of references that use Point Click Care (PCC) and Electronic Health Record (EHR) integration. Oral Presentations/Demonstrations may be conducted through videoconferencing or by conference call, if it is in the best interest of the Commonwealth to do so. This is the opportunity for the vendor to present and demonstrate the solution and to answer questions or to clarify the understanding of the evaluation committee in accordance with the requirements of this RFP. The Commonwealth reserves the right to reject any or all proposals in whole or in part based on the oral presentations/demonstrations. If required, the top 2 highest ranking vendors may be invited.
