Prospective Offerors:

This amendment is being issued to amend certain information in th e above named RFP.
All information contained herein is binding on all Offerors who respond to this RFP. Specific
parts of the RFP have been amended. The changes are listed below; new language has been
double underlined and marked in bold (i.e., word ), and language that has been deleted has been
marked with a strikethrough (i.e. word ).

1. REVISE Key Information Summary Sheet AS FOLLOWS:

Closing Date/Time: 3:00 PM; Friday November 9 Monday December 10, 2012

2. REVISE Section 1.6 (Closing Date) AS FOLLOWS:

An original, to be so identified, and five (5) copies of the Technical Proposal and
Financial Proposal (See Section IV) must be received by the Procurement Officer (ref.
Section 1.2) by 3:00 PM, Friday November 9 Monday December 10 , 2012 in order to
be considered.

3. REVISE Section 4.1 (Two Volume Submission) AS FOLLOWS:

4.1 Two Volume Submission

An original, to be so identified, and five (5) copies of the Technical Proposal and
Financial Proposal (See Section IV) must be received by the P rocurement Officer (ref.
Section 1.2) by 3:00 PM, Friday November 9 Monday December 10 , 2012 in order to
be considered.

4. The List of Attendees at the Pre -Proposal Conference and a Summary of the
Conference is included with this Amendment.

Offerors are reminded to acknowledge receipt of t his Amendment in the Transmittal Letter
accompanying the Proposal, as stated in Sections 2.2 and 4.2.A of the RFP. If you require
